Your doctor does not have to provide documentation, although having a doctor who is willing to work with you can be invaluable.

To make the disability decision the SSA will review the entirety of your medical file which can include not only doctor's notes but laboratory findings, Xrays, MRI, physical therapy notes and blood tests.

If your primary care doctor is willing to work with you, you should have them complete a RFC or residual functional capacity form and make sure the SSA has a copy. Google this form online and get a copy for your doctor. This form will clearly state your functional limitations to work.

If your doctor is listing information in your medical file that will hurt your claim or if they have clearly stated they are against you filing for disability, it may be time to get another doctor.
